Ankle Conditions
Ankle conditions are rated under 38 CFR § 4.71a, Diagnostic Codes 5270–5274. DC 5270 covers ankylosis; DC 5271 covers limited dorsiflexion (upward motion); DC 5272 covers limited plantar flexion (downward motion); DC 5274 covers instability.
VA Rating Levels
Limited dorsiflexion to 10 degrees, or plantar flexion to 10 degrees, or slight instability. Mild limitation affecting walking on uneven surfaces or prolonged standing.
Limited motion or moderate instability significantly affecting ambulation; unable to run, difficulty on slopes.
Unfavorable ankylosis in plantar flexion or severe instability. Major limitation in walking.
Ankylosis in plantar flexion greater than 40 degrees — severe restriction requiring adaptive footwear or assistive device.
Exam Tips & Key Evidence
- →Dorsiflexion and plantar flexion are measured separately — poor motion in both can yield separate ratings
- →Ankle instability after repeated sprains or ligament damage is separately ratable under DC 5262/5274
- →Document all falls and near-falls caused by ankle instability
