Chronic Hives (Urticaria)

Chronic hives (urticaria) are rated under Diagnostic Code 7825. Ratings are based on the level of treatment needed to control the condition. For VA purposes, the hives must be chronic, meaning they must occur at least twice per week for at least 6 weeks without any treatments. The schedule considers three lines of treatment in order of escalation.

VA Rating Levels

10%

Requires first-line treatment (antihistamines) for control.

30%

Requires second-line treatment for control, such as corticosteroids, sympathomimetics, leukotriene inhibitors, neutrophil inhibitors, or thyroid hormone.

60%

Requires third-line treatment for control (such as plasmapheresis, immunotherapy, or immunosuppressives) due to ineffectiveness of first and second-line treatments.

Exam Tips & Key Evidence

  • To qualify as chronic for VA purposes, hives must occur at least twice per week for at least 6 weeks without treatment.
  • The rating focuses on the level of treatment needed, not the severity of symptoms. Higher-level medications mean a higher rating.
  • Keep records of every medication prescribed for your hives and whether it was effective. This establishes the treatment escalation history.
  • If antihistamines alone are not controlling your hives, discuss second-line treatments with your doctor and document the medical necessity.
