Crohn's Disease / Ulcerative Colitis

Crohn's disease and ulcerative colitis (inflammatory bowel diseases) are rated under 38 CFR § 4.114, Diagnostic Code 7323. The VA evaluates symptom frequency, blood and stool abnormalities, anemia, and nutritional impact.

VA Rating Levels

10%

Mild — occasional exacerbations with some abdominal distress, loose stools, or mild anemia. Can manage daily activities with medication.

30%

Moderate — frequent exacerbations with intermittent diarrhea, abdominal pain, and some malnutrition or anemia.

60%

Moderately severe — frequent exacerbations with persistent diarrhea, abdominal pain, weight loss, and moderate anemia. Significant impairment of health.

100%

Severe — pronounced impairment of health with marked weight loss, anemia, and nearly constant exacerbations. May require parenteral nutrition or colectomy.

Exam Tips & Key Evidence

  • IBD can be primary or secondary to PTSD, NSAIDs, or medication side effects
  • Hospitalizations and surgical procedures are strong evidence for higher ratings
  • Document weight loss, anemia values, and number of bowel movements per day
