Hand and Finger Conditions

Hand and finger conditions are rated under 38 CFR § 4.71a. Ratings depend on which finger, dominant vs. non-dominant hand, and degree of limitation or ankylosis. Thumb and index finger losses carry the highest ratings due to their functional importance.

VA Rating Levels

10%

Favorable ankylosis of thumb at MP joint or minor limitation of motion in index/middle finger. Mild functional impact.

20%

Unfavorable ankylosis of the thumb or favorable ankylosis of two or more digits. More significant grip or pinch impairment.

30%

Loss/unfavorable ankylosis of multiple fingers or the thumb and another digit. Major loss of hand function.

40%

Amputation or functional equivalent at the index finger proximal to the third joint, dominant hand.

60%

Amputation or functional equivalent of the thumb at the metacarpophalangeal joint, dominant hand.

Exam Tips & Key Evidence

  • Each finger can be rated separately — document each one affected
  • Cold injury residuals (frostbite) affecting fingers are rated separately but still under musculoskeletal codes
  • Grip strength and pinch strength tests can support higher ratings
