Hernia (Inguinal, Ventral, Umbilical, Femoral, Incisional)

Hernias (excluding hiatal) are rated under DC 7338 based on the size of the hernia, whether it is repairable, and the level of pain experienced during daily activities. You cannot receive multiple separate ratings for multiple hernias under this code. However, if both inguinal canals are herniated and each would rate at least 10%, the higher rating gets an additional 10% added. If service records note an inguinal hernia, service connection is generally warranted without a medical opinion.

Irreparable hernia present for 12 months or longer, smaller than 3 cm.

Irreparable hernia 3 cm to less than 15 cm, present 12 months or longer, with pain during at least one activity: bending over, daily living activities, walking, or climbing stairs.

30%

Irreparable hernia 3 cm to less than 15 cm, present 12 months or longer, with pain during at least two of those activities.

60%

Irreparable hernia 15 cm or greater, present 12 months or longer, with pain during at least two activities.

100%

Irreparable hernia 15 cm or greater, present 12 months or longer, with pain during at least two activities and significant functional impairment.

Exam Tips & Key Evidence

  • If your service records document a hernia, that typically qualifies for service connection without needing a separate medical opinion. Make sure those records are in your file.
  • Hiatal hernias are not rated under this code. They are rated under the GERD/difficulty swallowing schedule instead.
  • The size of the hernia and which activities cause pain are both important. Be specific with your examiner about what movements and daily tasks cause you pain.
  • If both inguinal hernias are present and each would rate at least 10%, you get a 10% bonus added to the higher rating. This is a unique rule specific to bilateral inguinal hernias.
  • You cannot get separate ratings for multiple hernias under this code. However, under the updated 2024 schedule, some previously combined conditions may now qualify for separate evaluations.
