Hypothyroidism (Underactive Thyroid)

Hypothyroidism is rated under DC 7903. This condition occurs when the thyroid produces too few hormones. Unlike most conditions, hypothyroidism does not follow a permanent rating scale. The VA assigns a 30% temporary rating for 6 months after initial diagnosis, then rates based on residual symptoms. If the veteran develops myxedema, a severe and potentially life-threatening complication, a 100% rating is assigned until a physician determines it is no longer life-threatening, followed by an additional 6 months at 100%, and then re-evaluation for residuals. Myxedema symptoms include cold intolerance, constipation, dementia, depression, heart issues, muscle cramps & weakness, weight gain, tiredness, and slowed thinking.

30%

Assigned for 6 months after initial diagnosis of hypothyroidism without myxedema. After 6 months, the veteran is rated on residual symptoms.

100%

Assigned during active myxedema. Continues for 6 months after a physician determines the myxedema is no longer life-threatening, then the veteran is rated on residual symptoms.

Exam Tips & Key Evidence

  • Hypothyroidism is a presumptive condition for veterans exposed to Agent Orange.
  • After the initial 30% temporary rating, you will be rated on residual symptoms. Common residuals include fatigue, weight gain, cold intolerance, depression, cognitive difficulties, muscle weakness, and constipation. Document every symptom you experience.
  • If your medication (typically levothyroxine) controls symptoms well with no residuals, the VA may reduce your rating after the 6-month period. Make sure your records reflect any ongoing issues.
  • Common secondary conditions include eye problems (blurred or double vision), digestive conditions, and mental health disorders. Each of these can potentially be rated separately.
  • If your effective date of service connection falls after the 6-month window from initial diagnosis, you cannot receive the temporary 30% and will go straight to a residuals-based evaluation.
