Kidney Stones (Nephrolithiasis)
Kidney stones are rated under DC 7508 and DC 7509 (hydronephrosis). Kidney stones are presumptive if diagnosed within one year of separation at a compensable level. Most kidney stones are rated under the hydronephrosis schedule (DC 7509), which ranges from 10% to 30% based on frequency, infection, and kidney function impairment. Stones requiring frequent invasive or non-invasive procedures more than 2 times per year can rate at 30% under DC 7508. 'Frequent stones' is defined as more than one per year.
VA Rating Levels
Occasional kidney stones that do not result in infection or need for catheter drainage (rated under hydronephrosis, DC 7509).
Frequent kidney stones (more than one per year) requiring catheter drainage (rated under hydronephrosis).
Frequent stones with infection (pyonephrosis) and impaired kidney function, OR frequent stones requiring invasive or non-invasive procedures more than 2 times per year.

- →Kidney stones are presumptive if they appear within one year of separation at a compensable level. Document the onset date carefully.
- →'Frequent stones' means more than one per year. Keep detailed records of every stone event, including ER visits, imaging, and any procedures.
- →Invasive procedures include open surgical lithotomy, PCNL, and RIRS. Non-invasive procedures include shockwave lithotripsy and ureteroscopic retrieval. Both count toward the procedure frequency threshold.
- →If kidney stones cause severe kidney impairment, the condition can be rated under the kidney failure schedule instead, if it produces a higher rating.
- →Hydronephrosis (excess fluid buildup in the kidney from urine backup) is commonly caused by stones and has its own rating scale. Discuss with your VSO which schedule best captures your symptoms.
