Knee Conditions

Knee conditions can be rated under multiple diagnostic codes simultaneously. Common codes include 5003 (arthritis), 5257 (instability), 5260 (limited flexion/bending), and 5261 (limited extension/straightening). You can receive separate ratings for different knee problems.

VA Rating Levels

10%

For limited flexion (bending): knee bends to at least 45 degrees but not more. For limited extension (straightening): leg extends to within 10 degrees of straight. For instability: slight recurrent subluxation or lateral instability. For arthritis: X-ray evidence with painful motion. Basically, your knee hurts and you have some limitation, but you can still get around.

20%

For limited flexion: knee only bends to 30 degrees. For limited extension: can't straighten past 15 degrees. For instability: moderate recurrent subluxation or lateral instability (your knee gives out noticeably). More significant daily limitations.

30%

For limited flexion: knee only bends to 15 degrees. For limited extension: can't straighten past 20 degrees. For instability: severe recurrent subluxation or lateral instability (your knee frequently gives out). Substantial difficulty with stairs, walking, standing.

40%

For limited extension: can't straighten past 30 degrees. This level of limitation significantly affects daily activities.

50%

For limited extension: can't straighten past 45 degrees. Your leg is basically stuck bent. Also the minimum rating for some prosthetic knee replacements.

Exam Tips & Key Evidence

  • You can get separate ratings for the same knee: one for instability (5257) and one for limited motion (5260/5261). This is called "pyramiding" and it's allowed for knees.
  • If both knees are affected, each is rated separately
  • Meniscal conditions (torn cartilage) are rated under DC 5258 (symptomatic removal: 10%) or DC 5259 (locking/effusion: 20%)
  • After a total knee replacement, you get a temporary 100% rating for one year, then a minimum of 30%
  • Always describe instability ("giving way") separately from pain and limited motion at your C&P exam
