Pleural Effusion / Pleurisy

Pleurisy and pleural effusion are rated under 38 CFR § 4.97, Diagnostic Code 6844. After active inflammation resolves, residuals such as pleural thickening, adhesions, or decrement in pulmonary function may yield a continuing rating.

VA Rating Levels

10%

Minimal pleural disease with small pleural thickening but no significant lung function reduction.

30%

Moderate pleural disease — thickening or adhesions with moderate reduction in lung expansion or recurring fluid.

50%

Advanced pleural disease — significant reduction in FVC, marked restriction, or persistent effusion.

Exam Tips & Key Evidence

  • Pleurisy can result from pneumonia, TBI blast injuries, or exposure to toxic substances
  • Once active phase ends, file for residuals including reduced pulmonary function
