Stenosis of the Larynx

Stenosis (narrowing) of the larynx is rated under DC 6520 based on FEV-1 results combined with a flow-volume loop showing upper airway obstruction. If the condition results in chronic inability to speak, it may alternatively be rated under the aphonia (loss of speech) code if that provides a higher rating. A permanent tracheostomy qualifies for a 100% rating.

FEV-1 less than 40% with a flow-volume loop showing upper airway obstruction, or requires a permanent tracheostomy.

Exam Tips & Key Evidence

  • Both FEV-1 results and a flow-volume loop test are needed for this rating. The flow-volume loop must specifically show an upper airway obstruction.
  • If your narrowed larynx also prevents you from speaking, you may get a higher rating under the loss of speech (aphonia) code. Discuss both options with your representative.
  • A permanent tracheostomy qualifies for an automatic 100% rating under this code.
  • This condition can result from intubation injuries, combat trauma, or surgeries. Establish the service connection to the original cause of the narrowing.
