Thoracic Spine (Mid-Back)

Thoracic spine conditions are rated under 38 CFR § 4.71a using the General Rating Formula for Diseases and Injuries of the Spine (same as lumbar spine). Forward flexion and combined range of motion are the key measurements.

VA Rating Levels

10%

Forward flexion greater than 60 degrees (thoracolumbar combined), or muscle spasm/guarding with tenderness but no abnormal gait.

20%

Forward flexion 30–60 degrees, or combined range of motion ≤120 degrees, or muscle spasm causing abnormal gait or spinal contour.

40%

Forward flexion 30 degrees or less, or favorable ankylosis of the thoracic spine.

50%

Unfavorable ankylosis of the entire thoracic or thoracolumbar spine.

100%

Unfavorable ankylosis of the entire spine.

Exam Tips & Key Evidence

  • Mid-back conditions are often rated together with lumbar spine — make sure each region is measured separately
  • Compression fractures from vertebral injuries can also be rated separately
  • Document radiculopathy into the abdomen or legs if present
